CONFIDENTIALITY
All records are kept confidential according to
the Health Insurance Portability and Accountability
(HIPPA) Privacy rule of 1996. According
to this policy, we are not allowed to release
medical information to anyone without
your written consent.
The program’s Data Manager will collect the
data and organize it. All staff members must sign confidentiality statements and attend confidentiality training.
GRIEVANCES
Problems and complaints should be addressed to the
Administrator Director of the HIV Team or the Grievance
Counselor at 312-572-2724. Remember to get
the names of anyone involved in an incident or problem,
the date and the approximate time.
